Before you start: Here’s what you’ll need to know
The information you get out of the Go Red Heart CheckUp is only as accurate as the information you put in. That’s why it’s important to have current readings on your total cholesterol, LDL (“bad”) cholesterol, HDL (“good”) cholesterol, triglycerides, blood pressure, fasting glucose, height, weight and waist circumference with you as you complete the questionnaire. If you don’t have those numbers from a recent physical exam, take the form below along to your next doctor’s visit and use it to record the information.
